Sylvio Alves f8d2e00a0e includes: remove duplicated entries in zephyr-tree 
Remove duplicated #include directives within the same
preprocessor scope across the Zephyr tree.

Duplicates inside different #ifdef branches are preserved
as they may be intentional.
